Our Privacy Promise to You

SurveyVitals is committed to protecting the information of current and former clients and their patients as described in this Privacy Promise. This Privacy Promise applies to the SurveyVitals’ portal, the application, website, and surveys.

Privacy Information is Confidential

SurveyVitals requires that client and patient information be held in strict confidence. Our contracts with our third-party service providers strictly limit the use of privacy information and require strong security protections. We currency use the following third parties:
  • Twilio for sending surveys. (We only share the relevant patient contact information- email and/or phone number).
  • AWS for data storage.

We Protect Your Information

All data is kept secure with encryption, passwords to protect our databases, and world-class virus/intrusion detection software in place.

We Collect Information to Serve You and Your Patients

Collecting information makes it possible for SurveyVitals to deliver the kind of personalized service you expect. Some ways in which we use this information include:
  • Gathering metrics about the survey taking experience
  • Making our site easier to use
  • Logging for security reasons
  • Improving our services to you.
  • Google Analytics on our main website page.
  • Gathering and processing information and/or contact requests from the main page of our website.

We Do Not Sell Information

This commitment is the foundation of our Privacy Promise to you. We do not share your information or the information of your patients with anyone other than those required to perform our services for you, and we hold our service providers to the same standards. We do not use the information we obtain for marketing to any Client’s patients, for any reason.

We Share the Information We Collect as Permitted by Law

Occasionally we need to share results or data with third parties. For example:
  • If a client instructs us to, we share data with third party registries and CMS.
  • If a patient leaves a comment indicating that they may harm themselves or others we will share with the client and necessary authorities.
  • If needed to comply with any applicable law, regulations, legal processes or enforceable government requests.

Updates and Effective Date

Any material changes to the Privacy Promise will be posted on this website. The effective date of the Privacy Promise is September 8, 2014. It was last updated on August 19, 2020.

Public Content

SurveyVitals creates white papers and reports using aggregated and de-identified survey results. We do not include identifying information about our clients or patients in these publications unless we have consent of the applicable parties. Patients may choose to publish a comment about their experience with their provider on either SurveyVitals.com, to google reviews, or on their own social media pages. These comments, and if applicable names, are intended for public consumption and are therefore viewable by the public.

Health Insurance Portability and Accountability Act of 1996 (HIPAA)

SurveyVitals, complies with the requirements of the Health Insurance Portability and Accountability Act of 1996 as amended by the American Recovery and Reinvestment Act of 2009 (“HIPAA”), and implementing regulations which are codified at 45 C.F.R. Parts 160, 162 and 164, as such regulations may be amended from time to time (collectively referred to herein as the “HIPAA Rules”). All Protected Health Information (PHI) shared by Clients for business purposes is protected in accordance with the HIPAA Rules through Business Associate Agreements (BAAs). SurveyVitals pledges to protect any PHI obtained until such time as that PHI is destroyed.
